1. Services Provided
The laboratory provides digital full-mouth dental implant restorations for dentists to deliver to their patients. These services include the design, fabrication, and delivery of full-mouth restorations.
2. Payment Terms

Advance Payment: The laboratory requires full payment before the delivery of any work. All payments must be received in full before the final product is shipped or delivered to the dentist.

Additional Fees: Each case includes up to two design revisions. Any additional design revisions requested beyond the included two will incur additional fees, which will be outlined in the corresponding invoice.
3. Limitation of Liability

Workmanship Warranty: The laboratory guarantees the quality of the materials and workmanship of the restorations provided. In the event of a defect in materials or workmanship, the laboratory will repair or replace the defective restoration at no additional cost to the dentist.

Liability for Use: The laboratory is not liable for any damages, including financial loss, arising from the use of the restorations by the dentist or their patients. The dentist assumes full responsibility for the use of the products provided by the laboratory.

Indemnification: The dentist agrees to indemnify, defend, and hold harmless the laboratory from any and all claims, damages, liabilities, costs, and expenses, including legal fees, arising from or related to the use of the products by the dentist or their patients.
4. Case Acceptance

The laboratory reserves the right to accept or decline any case at its discretion.

All cases submitted must include accurate and complete patient data. Any errors or omissions in the provided data may result in delays, additional fees, or rejection of the case.
5. Revisions and Approvals

Each case includes up to two design revisions. Any further revisions requested by the dentist will be subject to additional charges, which will be detailed in an invoice before proceeding with the work.

The dentist must provide written approval of the design before the laboratory proceeds with fabrication. Any delays in approval may result in changes to the delivery timeline.
6. Delivery and Shipping

Delivery timelines will be estimated at the time of case acceptance. The laboratory will make every effort to meet the agreed-upon delivery date, but is not liable for delays due to circumstances beyond its control.

Shipping costs will be borne by the dentist unless otherwise agreed upon in writing.

8. Governing Law

This agreement shall be governed by and construed in accordance with the laws of the state in which the laboratory is located.

Any disputes arising under this agreement shall be resolved through binding arbitration in the jurisdiction of the laboratory’s location.
9. Amendments

The laboratory reserves the right to amend these terms and conditions at any time. Any changes will be communicated to the dentist in writing and will take effect immediately upon such notice.
